Ergonomic Home Evaluation Service

Our Ergonomic Home Evaluation Service is designed to help optimize your home workspace and reduce the risk of musculoskeletal issues. This comprehensive service includes a thorough workstation evaluation to assess your current setup and ensure it promotes comfort and productivity. We’ll identify any muscle or joint issues and recommend adjustments to improve your posture and reduce strain.

If needed, we offer workstation adaptations, such as adjusting furniture or recommending new ergonomic equipment like chairs, desks, or keyboards. We will also provide an on-site loan of equipment during the visit to test out new setups and determine their effectiveness.

As part of the service, we may also include a stretching or exercise program tailored to alleviate discomfort and improve mobility. For clients seeking new ergonomic chairs, we can arrange a visit to a supplier to find the right chair at a competitive price, ensuring comfort and support.

At the end of the evaluation, we provide a detailed consultation report with recommendations and any follow-up actions. A second visit is included to ensure that any changes made are effective, and a third visit is available if additional adjustments or equipment are needed.

This service is offered in a flexible, three-visit format:

  • Visit 1: 1 hour (Initial workstation evaluation and adjustments)
  • Visit 2: 1 hour (Follow-up and additional recommendations)
  • Visit 3 (Optional): 1 hour (If needed, for further adjustments or assistance with equipment purchases)

Please note that a minimum of two visits is required to ensure comprehensive support and lasting results.

Ergonomic Workplace Evaluation (Less Than 10 People)

The Ergonomic Workplace Evaluation Service is designed for small businesses with fewer than 10 employees. This evaluation aims to analyze and optimize your employees’ workstations to improve comfort, productivity, and reduce the risk of musculoskeletal issues.

The service begins with a workstation evaluation for each employee, assessing the layout of their equipment, the ergonomics of chairs and desks, and body alignment during work activities. We then conduct a musculoskeletal evaluation to identify any muscle or joint problems. Based on this assessment, we recommend workstation adaptations to improve comfort and reduce strain.

If necessary, we recommend ergonomic equipment suited to each employee’s needs and provide on-site equipment loans to test new setups during the visit. If requested, we can also accompany the client to purchase an ergonomic chair at a competitive price.

Additionally, we offer a stretching or exercise program tailored to reduce muscle tension and improve mobility. We also provide neuro-proprioceptive taping or sports taping as an option to help alleviate pain and improve posture.

At the end of the evaluation, we provide a consultation report with detailed recommendations for each workstation, along with follow-up actions if needed. A second visit is included to assess the effectiveness of the changes, and a third visit is available if further adjustments or new equipment are required.

If necessary, we will loan equipment on-site during the visit, allowing employees to test out ergonomic solutions in real time. Additionally, for employees seeking new ergonomic chairs, we can accompany them to a supplier to ensure they find the right chair at a competitive price.

Rates:

The pricing for this service is negotiable. The head occupational therapist will contact the employer directly to discuss and finalize the price. We offer the following options for payment:

  • Option 1: Employer covers 100% of the costs.
  • Option 2: Employees can use their insurance to cover the cost, with the remaining balance paid by the employer.

This service is tailored to meet the needs of large companies seeking to enhance the health and productivity of their workforce, ensuring that every employee benefits from a personalized ergonomic assessment.

Driving Assessment Service

The Driving Assessment Service is typically requested by clients seeking to regain their driver’s license, often following a request from the SAAQ (Société de l’Assurance Automobile du Québec). This service evaluates the functional and cognitive abilities required for safe driving.

Evaluation 1: Functional and Cognitive Assessment

During the first evaluation, we assess several key aspects necessary for driving, including the client’s physical strength, vision, memory, and attention. These components are essential to ensuring the client is capable of driving safely and efficiently. This evaluation can take place either at the client’s home or in a clinic, based on their preference. Cognitive or physical training recommendations can be offered to maximize your potential in the cognitive or perceptual spheres deemed to be less efficient.

Evaluation 2: Practical On-Road Assessment

The second evaluation involves a practical on-road assessment conducted at a driving school. This session directly tests the client’s driving skills in a controlled environment. It includes tests on reaction time, adherence to traffic rules, and real-time decision-making abilities.

At the end of this assessment, there are 3 possibilities: Pass (what we wish for you), Fail (dangerous driving) and finally Fail with learning abilities (refresher courses will be required).

Duration and Requirements

  • Evaluation 1: 2.5 to 3 hours (at home or in a clinic)
  • Evaluation 2: 1 hour (at a driving school)

Note: Both evaluations must be completed to finalize the Driving Assessment Service.

Car Adaptation and Training for Adaptive Driving

This service is designed to assist clients with disabilities in modifying their vehicles to ensure safer and more comfortable driving. It is often requested by the SAAQ.

Step 1: Functional and Cognitive Evaluation

In the first session, we assess the client’s physical abilities, cognitive functions, and overall readiness to drive. This includes evaluating strength, coordination, attention, and problem-solving skills. This evaluation can take place at the client’s home or in a clinic and lasts approximately 2.5 to 3 hours.

Step 2: On-Road Evaluation

The second session involves a practical driving assessment at a driving school. During this session, the client tests their skills in an adapted vehicle under controlled conditions. This includes assessing their reaction time, adherence to traffic laws, and confidence behind the wheel. The session lasts 1 hour.

Together, these evaluations ensure that the vehicle modifications align with the client’s needs and that they are equipped to drive safely in their adapted vehicle. Both evaluations are required to complete the service.

Step 3: Adapted driving training sessions. Note that the SAAQ covers up to $500 of these costs in an adapted vehicle.

Step 4: Validation of safe driving with the adapted vehicle

Step 5: Registration for the financial assistance program to cover adaptation costs.

Step 6: Adaptation of the vehicle and validation of adaptations.

Home Care After Surgery and Before Starting Physiotherapy

This service focuses on optimizing post-operative recovery and preparing clients for future physiotherapy or occupational therapy.

Functional Assessment
During the first visit, we perform a detailed assessment of the client’s physical capabilities and their ability to perform daily activities (ADLs).

Exercise Program
We create a personalized exercise program to enhance strength, mobility, and flexibility, setting a strong foundation for upcoming rehabilitation.

Advice on Assistive Devices
We provide guidance on selecting and using assistive devices such as walkers, canes, or specialized utensils to support independence and safety at home.

Duration and Structure

  • Visit 1: 1 hour for the initial assessment and program setup.
  • Subsequent Visits: 45 minutes to monitor progress and adjust the care plan as needed.

This service ensures a smoother transition to clinic-based therapies while promoting recovery and independence at home.

Home Adaptation Program (HAP)

This service is part of a fully funded government program aimed at making homes safer and more accessible for individuals with specific needs.

Evaluation 1: Architectural Modifications Assessment

In the first session, we evaluate the client’s home to determine what architectural changes are necessary to improve safety and accessibility. This session lasts over 2 hours and involves detailed planning.

Evaluation 2: Meeting with the HAP

The second session includes a 90-minute meeting with representatives of the Home Adaptation Program to review the proposed modifications and discuss funding options.

Evaluation 3: Follow-Up Meeting

After validation from the HAP, we conduct a final meeting with the client to confirm the modifications and next steps.

Funding and Process

This program includes up to $1,250 in refunds from the HAP to cover the cost of evaluations and services. Clients must complete all three sessions to be eligible for the reimbursement. We facilitate communication with the HAP to ensure grant approval and successful implementation of the changes.

Adapted Transport (Paratransit) Application

This service facilitates access to specialized transportation for individuals with reduced mobility or disabilities.

Functional Assessment

We perform a detailed evaluation to assess the client’s mobility and ability to use adapted transportation safely and effectively.

BERG Balance Test

This test measures balance and assesses the client’s risk of falling, helping determine their eligibility for paratransit services.

Duration

The evaluation is conducted at the client’s home and lasts 1 hour.

This service ensures that clients receive the support they need for safe and accessible transportation tailored to their specific requirements.

Disabled Parking Permit Application

This service supports individuals with reduced mobility or disabilities in obtaining a disabled parking permit, whether for a designated parking space or a vehicle sticker.

Functional Assessment

We evaluate the client’s mobility to determine their eligibility for a parking permit.

BERG Balance Test

This test assesses the client’s balance and evaluates their risk of falling, ensuring the need for accessible parking is justified.

Duration

The evaluation is conducted at the client’s home and lasts 1 hour.

This service ensures that clients receive the appropriate support and documentation needed for accessible parking.

Ability Assessment Certificate

This service provides a comprehensive evaluation of a person’s ability to perform daily activities, determining their functional independence.

Evaluation and Report

The evaluation involves assessing the client’s skills in carrying out everyday tasks, and a detailed report is provided based on the findings.

Duration

The assessment typically takes approximately 2 hours.

This service is ideal for individuals who require an official certificate for their ability to function independently or to support other medical or social service applications.

Disability Tax Form

This service helps individuals complete the necessary forms to apply for the disability tax credit.

Evaluation and Report

A full functional assessment is conducted to evaluate the client’s needs, followed by the completion of the required disability tax form.

Duration

The evaluation takes 60 minutes, with an additional 30 minutes to complete the report.

This service ensures that clients receive professional assistance in securing the disability tax credit with the necessary documentation.

Sensory Rehabilitation

This service focuses on diagnosing sensory impairments and providing exercises to help improve sensory function.

Initial Assessment at the Clinic

The initial assessment involves a thorough evaluation of the client’s sensory abilities and challenges, followed by the development of a tailored rehabilitation plan.

Follow-up Sessions

Follow-up visits are scheduled once every three weeks to monitor progress and adjust sensory exercises based on the client’s needs.

Duration and Rates

  • Initial Evaluation: 120 minutes at $200
  • Reevaluation and Exercise Adjustment: 90 minutes at $180

This service helps clients improve their sensory processing and function, contributing to overall well-being and independence.
