Nerve Adhesions
A nerve that has lost its ability to glide freely within its surrounding tissues can remain irritated, even when there is no longer active compression.
This mechanism cannot be seen on an MRI and can only be identified through a clinical assessment.
The Role of Nerve Sensitization in Persistent Pain
Chronic pain is rarely “all in your head,” but it does involve the brain.
Understanding this mechanism can fundamentally change the way sciatica is treated.
Peripheral Sensitization
Peripheral sensitization occurs when the nerve itself becomes overly excitable after being compressed for an extended period.
Even after the mechanical pressure has been reduced, the nerve may continue sending pain signals in response to stimuli that would not normally be painful.
Central Sensitization
Central sensitization goes further.

Piriformis Syndrome: A Frequently Underdiagnosed Cause
The piriformis is a deep, pear-shaped gluteal muscle that connects the sacrum to the greater trochanter of the femur.
It plays a role in the external rotation of the hip.
Its anatomy is particularly important because the sciatic nerve passes either directly beneath it or, in some people, through its muscle fibres.
When the piriformis is in spasm, inflamed, or shortened, it can directly compress the sciatic nerve. This causes deep buttock pain that radiates down the leg, much like a herniated disc.
Signs that may point toward this diagnosis include:
- deep buttock pain that worsens with prolonged sitting;
- significant tenderness when the piriformis muscle is palpated;
- symptoms that are not explained by normal lumbar imaging.
This is where much of the confusion occurs.
The back MRI shows no clear problem, the patient receives no explanation, and the sciatica continues. Piriformis syndrome does not leave visible signs on standard lumbar imaging.
However, when it is correctly identified, treatment can be highly effective.
Targeted myofascial release of the piriformis, combined with specific stretches and rehabilitation of the pelvis and hip, can relieve the nerve compression at its source.
Once the correct diagnosis has been established, clinical results are often positive and relatively fast.

Sciatica that does not respond to conventional treatments requires a more advanced assessment.
In specialized physiotherapy, the approach is fundamentally different from a generic treatment protocol.
Treatment begins with a complete neuromuscular assessment.

- an increasing impact on sleep, work, and quality of life.
